Please define "Additional Items" for me.

Can anybody please define "additional items" for me. Why do I have to pay additional shipping cost per item of the same sale? Before I understand additional items being items from other sales combined with this sale. And does this not fall under "hidden costs"? It is a single sale why do I have to pay multiple shipping charges?
